With more choices in sensor integration, patients can start on Omnipod 5 without changing from their preferred continuous glucose monitor (CGM) brand. With this integration, users will benefit from tubeless automated insulin delivery along with:

  • The ability to start, deactivate, and view sensor glucose data directly from the Omnipod 5 Controller.

How to get patients started:

  • The Pods compatible with the FreeStyle Libre 2 Plus Sensor have a new NDC/NRC and require a new prescription. Intro Kit: NDC/NRC: 08508-3000-88 Pods: NDC/NRC: 08508-3000-42.
  • For new to Omnipod 5 patients, send both Intro Kit and refill Pods to your patients’ preferred pharmacy or ASPN Pharmacies' New Jersey location.
  • For your existing Omnipod 5 patients, send a prescription for FreeStyle Libre 2 Plus compatible refill Pods to your patients’ preferred pharmacy. Should they need any support, please have them contact the Omnipod Customer Care team at 1-800-591-3455.

How to Rx

The Omnipod 5 Automated Insulin Delivery System is indicated for use by individuals with type 1 diabetes mellitus in persons 2 years of age and older and type 2 diabetes mellitus in persons 18 years of age and older. The Omnipod 5 System is intended for single patient, home use and requires a prescription. The Omnipod 5 System is compatible with the following U-100 insulins: NovoLog®, Humalog®, Admelog®, and Kirsty®. 

WARNING: SmartAdjust™ technology should NOT be used by anyone under the age of 2 years old. SmartAdjust™ technology should also NOT be used in people who require less than 5 units of insulin per day, as the safety of the technology has not been evaluated in this population.
