Succesvol aan de slag met Omnipod® 5

In onderstaande informatie vindt u de belangrijkste stappen die uw patiënten kunnen nemen voor een succesvolle start met Omnipod 5.

 

Voorbereiding op de training

Indien uw patiënt Dexcom G6 gebruikt

  • Voorafgaand aan de training over het Omnipod 5-systeem dient uw patiënt gestart te zijn met Dexcom G6 met behulp van de Dexcom G6-app op een compatibele smartphone‡
  • Als uw patiënt momenteel de Dexcom-ontvanger gebruikt, moet hij/zij ook de Dexcom G6-app voor iOS of Android downloaden. OPMERKING: De Dexcom G6-ontvanger kan niet worden gebruikt als onderdeel van het Omnipod 5-systeem.

 

Indien uw patiënt FreeStyle Libre 2 Plus gebruikt

  • Verzeker u ervan dat uw patiënt training heeft gekregen over het gebruik van de FreeStyle Libre 2-sensor.
  • Zorg ervoor dat u uw patiënt een voorschrift heeft gegeven voor de FreeStyle Libre 2 Plus-sensor. Deze dient ongeopend meegenomen te worden naar de Omnipod 5-training‡.

De Dexcom G6 dient afzonderliijk aangevraagd te worden en er is een aparte machtiging vereist. De FreeStyle Libre 2 Plus kan uitsluitend in combinatie met Omnipod®5 aangevraagd worden en vereist een aparte machtiging. De Dexcom G6-sensor moet worden gebruikt met de mobiele Dexcom G6-app. De Dexcom G6-ontvanger is niet compatibel.

 

Startproces

Zodra de aanvraag van uw patiënt door Insulet is verwerkt, ontvangt hij/zij een e-mail met als onderwerp: "Voltooi nu het startproces van de Omnipod® 5".

Het is belangrijk dat het startproces van de Omnipod 5 wordt voltooid voordat de Omnipod 5-producttraining begint. 

Een patiënt dient het volgende te doen: 

  • Klikken op de link in de e-mail en inloggen met zijn/haar Omnipod-ID. Als uw patiënt nog geen Omnipod-ID heeft, kan hij/zij er een aanmaken via de link op het inlogscherm
  • De patiënt dient zijn/haar Omnipod-ID en wachtwoord te onthouden. Deze wordt gebruikt om tijdens der training in te loggen op de Omnipod 5-controller
  • Als de wettelijk voogd een account aanmaakt voor een afhankelijk persoon, dient deze een Omnipod-ID voor deze afhankelijke persoon te maken
  • Patiënten dienen de toestemmingsschermen waar informatie over de verwerking van persoonlijke gegevens wordt beschreven te bekijken en goed te keuren

    Het Omnipod 5-startproces is voltooid wanneer het bevestigingsbericht te zien is.

 

Verbinden met Glooko®

Glooko is het Omnipod 5-gegevensbeheerplatform waarop gebruikers:

  • Glucose- en insulinegegevens kunnen inzien
  • Real-time gegevens kunnen delen ter ondersteuning van geïnformeerde systeemaanpassingen
    • Wij raden aan dat patiënten hun Omnipod-ID koppelen aan hun Glooko-account. Als ze geen Glooko-account hebben, kan deze tijdens de installatie aangemaakt worden.
    • U kunt patiënten de ProConnect-code van uw kliniek geven om hun Glooko-rapport met uw team te delen.

Als deze stappen zijn voltooid, is uw patiënt klaar om aan de slag te gaan met de Omnipod 5-producttraining!

Producttraining

Als uw patiënt momenteel wijzigingen ondergaat in zijn huidige behandeling, inclusief een aanpassing van de insulinetherapie, zorg er dan voor dat hij/zij goed op de hoogte is van deze wijzigingen voordat hij/zij start met Omnipod 5.

 

Omnipod 5-starterskit

  • Tijdens de training of dit nu thuis of in het ziekenhuis plaatsvindt, dient de patiënt de, Omnipod 5-starterskit en enkele Omnipod 5-pods aanwezig te hebben. Al deze materialen zal naar het huisadres worden verstuurd van de patient. Ook dient de patiënt een flacon snelwerkende insuline* die hij/zij gebruikt mee te nemen.

Als uw patiënten de levering van de Omnipod 5-starterskit verwacht en deze niet binnen 3 dagen voor de geplande training heeft ontvangen, neem dan contact op met het Klantenserviceteam op 0800 0229512* of +31207989337 vanuit het buitenland.

Wanneer Omnipod 5 Pods niet binnen 3 dagen voor de geplande training zijn ontvangen, dienen patiënten contact op te nemen met zijn hulpmiddelen leverancier.

 

Sensors

Dexcom G6-sensor

  • Patiënten dienen naar de training te komen met een actieve Dexcom G6-sensor met gebruik van de Dexcom G6-app op een compatibele smartphone en ervoor te zorgen dat de Dexcom G6-ontvanger is uitgeschakeld.

FreeStyle Libre 2 Plus-sensor

  • Als ze momenteel een FreeStyle Libre 2 Plus-sensor gebruiken, mogen patiënten deze sensor blijven dragen tijdens hun Omnipod 5-training. Daarnaast moeten patiënten een nieuwe, ongeopende FreeStyle Libre 2 Plus-sensor meenemen naar de Omnipod 5-training.

 

Insuline

  • Patiënten dienen niet te vergeten om een flacon snelwerkende insuline* mee te nemen naar de training.
*NovoLog®/NovoRapid®, Humalog®, Trurapi®/Insuline aspart Sanofi®, Kirsty® en Admelog®/Insuline lispro Sanofi® zijn compatibel met het Omnipod 5-systeem voor gebruik gedurende maximaal 72 uur (3 dagen).

Ondersteuning na de training

Om het meeste uit Omnipod 5 te halen, moedigen we uw patiënt aan om voorafgaand aan de producttraining onze instructievideo's te bekijken. Deze en andere handige hulpmiddelen zijn te vinden op de pagina Omnipod 5 instructievideo's

Step 1

Pre-training

Dexcom

Prior to your patient starting on the Omnipod 5 system, they will need to be using Dexcom G6 with the Dexcom G6 app on a compatible smartphone. If your patient is using the Dexcom receiver, they must download the Dexcom G6 app for iOS or Android.

Omnipod 5 Setup

Once the Omnipod 5 order has been approved by Insulet, your patient will receive an email from Insulet with a link to complete their online Omnipod 5 setup. This can be completed via a computer or mobile phone.


Your patient will need to complete online Omnipod 5 setup before their Omnipod 5 product training. This will involve signing in with, or creating an Omnipod ID and completing the consent screens, where we provide your patient with information on the processing their personal data.

Glooko

Through the Omnipod 5 setup process, we recommend that your patients link their Omnipod ID to their Glooko Account. A Glooko account can be created at this stage.

Glooko will enable your patient to:-

  • See their glucose and insulin data.
  • Share their diabetes data with you, so please share your clinics Proconnect code.

 

Preparation for Training

Encourage your patient to watch the ‘How to Videos’ here to prepare them for their product training.

Expectation setting

In line with other AID Systems, it is important you provide your patients with realistic expectations before starting on Omnipod 5.

Your patients can start immediately in Automated Mode but it may take a few weeks for full optimisation to be achieved. Omnipod 5 System continues to adapt to your patient’s needs as it builds history on insulin requirements.

Prior to initiation, please ensure your patients are familiar with the following requirements :

✓ Need to deliver bolus for carbohydrate intake at meals and snacks to minimise postprandial hyperglycaemia.
✓ Need to perform Pod and sensor changes.
✓ Need to respond to alerts and alarms.

Step 2

Product training

Pre-start therapy guidance

Please provide guidance regarding any changes to current therapy in preparation to starting on Omnipod 5.

Dexcom

Your patient will need to come to training wearing their active Dexcom G6 sensor and using the Dexcom G6 app. Please request their Dexcom G6 receiver is switched off.

Starter kit

Ensure you have the Omnipod 5 starter kits and box for Omnipod 5 Pods available if product training is taking place at the hospital. The Omnipod 5 Starter kit and Omnipod 5 Pods will be sent to your patient’s home if a virtual home start is planned. Remember to ensure a vial of compatible rapid U-100 acting insulin is prescribed*.
